Step 3: How to Enroll


It's easy. Just call the toll-free Medicaid Options HelpLine at 1-888-367-6554.
 
  Have the Medicaid ID or Gold Card Number, the Social Security Number (last four numbers only) and birth date ready for each person you are enrolling.
  Contact your Department of Children and Families office if you do not know the Medicaid ID number, need a new card or to find out if you or the person you want to enroll are on Medicaid.
  Report address, phone, or caretaker/payee changes to your caseworker.
 
Exemptions
 
If you answer YES to any of the following questions, you cannot choose a Medicaid managed care plan.
 
  Are you in a Medicare Health Maintenance Organization (HMO)?
  Are you in a nursing home, mental hospital, or a long-term care facility?
  Are you receiving hospice services?
  Are you in the share of cost (medically needy) program?
  Are you covered by other major health insurance?
 
After you enroll in a Florida Medicaid health care plan or the State assigns you to a plan, you will have 90 days after you are enrolled to try your plan, and change plans, if you want. At the end of 90 days you will stay in your plan for the next nine months before you can change plans again.
